Welcome to highlandfamilymedicine.com (the "Site"), operated by Highland Family Medicine ("Highland Family Medicine," "we," "us," "our," or "the Practice"). By using this Site, you agree to these Terms of Service. Please read them, along with our Privacy Policy, before submitting the appointment-request form.
1. What This Site Is
This Site is a marketing and appointment-request tool for Highland Family Medicine, a primary-care practice with two locations in Murfreesboro, Tennessee. Submitting the form on this Site sends a request to our staff — it does not book, confirm, or guarantee an appointment. A member of our team will contact you, typically by phone, to confirm the date and time.
2. Medical Emergencies
This Site and its form are not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. This form is for non-emergency appointment requests only. Submitting a request through this Site does not create an urgent-care or emergency-response channel to our office, and we cannot guarantee any specific response time.

You agree to provide accurate contact information (name, phone, and, if given, email) so that our staff can reach you to confirm your request. We are not responsible for our inability to reach you if the contact information you provided is incomplete or incorrect.
6. How Your Information Is Handled
Information submitted through this form is handled according to our Privacy Policy, which describes what we collect, how we use it, and which outside services (our CRM, our email-delivery provider, and our website analytics) process it on our behalf.
